Walburga didn't know anything about Regulus being trans until he came home with short hair at Christmas his first year at hogwarts. He got on the train as her daughter and got off it as her son. He cut his hair the second he was on that train and borrowed trousers from Sirius. Obviously Walburga wasn't happy but everyone already knew Regulus was a guy now so she had to play along with it. She never called him Regulus though, she never called him her son, to Walburga he was just trying to defy her.

James always asked Regulus to read the poems he wrote, he got shut down every single time. When Regulus died he left all his poetry notebooks for James. Some of the poems were just random things about life, most were about James. They started from Regulus' first year at Hogwarts until the day before he died. Lily came home that day to see James sobbing surrounded by notebooks.
